Summarise the record without assuming an outcome
Use this template when reviewing a recovery period or preparing information for an authorised colleague. It looks back at what was recorded. The discharge handoff looks ahead to the approved plan; the follow-up tracker keeps the next open action visible.
Separate the owner's report, the clinic's response and any assessment made by the treating clinician. A submitted check-in, a completed care task or an exported file does not establish that the patient has recovered. A missing update is a gap in the information, not a reassuring result.
Choose the period covered and identify the underlying records. A summary can be prepared while a question or appointment arrangement remains open. Keep those actions explicit rather than closing the recovery workflow to make the document look complete.
Copy the blank recovery summary
Complete the template in your clinic-approved system. Check the source records and ask the clinical reviewer to approve any clinical interpretation before sharing the summary with an authorised recipient. Use your browser’s Print command for a paper copy.

Keep the source records available through the clinic's normal process. Summarise with attribution: an owner reported something, a staff member sent a reply, or a clinician documented an assessment. Do not turn one type of entry into another when shortening it.
If a follow-up appointment is not yet scheduled, say so and name the role arranging it. If the source record contains no clinical assessment for this period, keep that limitation visible and route any assessment question to the treating clinician.

A complete fictional recovery summary
Training Clinic E is documenting two practice sessions for Practice Case F. The example contains administrative events only, with no actual patient information, medical assessment or private access link.
- Case and period
- Practice Case F; supervised practice sessions A and B, with session order recorded in the training record
- Prepared by and purpose
- Coordinator E prepares a draft after session B for the training handover review
- Plan and source records
- Sample Plan E, version 1; practice updates A and B and reply A remain in the approved training record
- Owner-reported information
- In update A, the staff member acting as the owner reports difficulty finding the clinic's reply. In update B, they report that the demonstrated route works
- Clinic responses and actions
- Reviewer E records an access acknowledgement and demonstrates where the practice reply appears; the coordinator records the result in the training record
- Clinical assessment
- None supplied by this nonclinical exercise; no recovery outcome is inferred
- Gaps or unavailable information
- No clinical observations or recovery media were collected in the access exercise
- Open actions and follow-up
- A fictional appointment arrangement is not yet scheduled. Coordinator E owns checking the approved booking process in the next practice session
- Review and record destination
- Training Reviewer F checks the draft against updates A and B; the approved version is retained in the authorised training record
- Sharing and transfer check
- Internal training use only. Reviewer F confirms where the approved summary is retained; the appointment action stays open with Coordinator E
The access question has a recorded answer, but the appointment arrangement still needs work. The summary states both facts. It does not label the case medically recovered or treat a prepared document as proof that every action is complete.
If the team corrects a source error or adds a later update, it follows the clinic's normal correction and version process. Staff keep the relevant history rather than silently replacing what a previous reviewer saw.
Check the summary against the source
An export is a separate file to handle through the clinic's records process. Producing or downloading it does not establish that a PIMS received it or that another staff member reviewed it. Check the transfer rather than assuming it happened.
- Confirm the case, period and approved plan version match the records being summarised.
- Check that owner reports, clinic communications and clinician assessments remain separately attributed.
- Identify missing information and keep unresolved actions assigned through the clinic's agreed process.
- Have the appropriate reviewer check the draft and confirm its approved record destination.
- If transferring an export, inspect the actual file, use an approved destination and confirm the intended record is available there.
Review and export the recovery activity
Vet Aftercare brings recorded owner updates, care-step activity and clinic follow-up into a recovery timeline. Clinic staff can export a recovery summary for authorised review or handover. Inspect the actual export and add any clinic-required context through your normal records process; this worksheet does not define or change the export's fields.
Vet Aftercare is a separate recovery workspace. Patient details are entered manually and recovery summaries do not automatically synchronise with your PIMS. The clinic remains responsible for its clinical assessment, record handling and decisions about further care.
