Use it for a single discharge
A reusable protocol explains the starting workflow. A discharge handoff records what the team has approved for one patient and what happens after the owner leaves. The follow-up tracker then records any unfinished review actions.
Complete and retain the handoff in your approved systems. This public page has no fields for entering patient details or private links. Keep the clinician's authoritative instructions and any patient-specific changes with the normal clinical record.
Copy the blank handoff record
Copy these fields into an approved local document. Replace the placeholders there before use and have your clinic approve the completed handoff. Use your browser’s Print command for a paper copy.

- Patient-specific care instructions
- [Treating clinician supplies or references the approved instructions here]
- Owner access and explanation
- [How the clinic shares private access and confirms the owner can use it]
- Owner questions to resolve
- [Questions requiring a response; record clinical answers in the approved clinical record]
- Expected updates
- [Clinician-approved questions, requested media if appropriate, and check-in windows with local time zone]
- Follow-up arrangements
- [Clinician-supplied appointment details, or not yet scheduled with a named person arranging it]
- Review responsibility and cover
- [Reviewing staff role, coverage periods, cover role and handover route]
- Urgent contact and record closure
- [Clinic-approved urgent/out-of-hours instructions; where the recovery summary will be retained]
Confirm that the patient-specific plan and the handoff refer to the same approved version. A missing appointment time should remain explicitly not scheduled until the responsible team confirms it.
Explain the review coverage and the clinic's urgent contact instructions. An owner update inbox must not be presented as an emergency service. Use the clinic's actual contact wording, without inventing availability.
You can use this document with your existing process. In Vet Aftercare, staff still enter patient details and prepare the plan manually; this template does not import automatically.

A complete fictional discharge handoff
Training Clinic A and Practice Case B are fictional. This example demonstrates the administrative record, with no treatment advice or actual access link.
- Case reference
- Practice Case B in the approved fictional training record
- Approved plan and version
- Training Plan A, version 1; approved sample wording retained in the fictional record
- Treating clinician approval
- Training Clinical Reviewer; approval recorded before the practice handoff
- Patient-specific care instructions
- The reviewer refers staff to the approved sample instructions in the fictional record. This example supplies no medical content.
- Owner access and explanation
- Coordinator A explains the private-plan access process to a staff member acting as the owner; no real invitation or message is sent
- Owner questions to resolve
- The person acting as the owner asks where to find the clinic's reply. Coordinator A demonstrates the process in the approved training environment.
- Expected updates
- The reviewer sets the sample questions and update window in the fictional plan; staff copy that agreed window into the handoff rather than choosing a clinical schedule
- Follow-up arrangements
- Not yet scheduled; Coordinator A owns confirming the arrangement through the clinic-approved route
- Review responsibility and cover
- Reviewer A checks during the agreed staffed period; Cover Reviewer B confirms acceptance of any unfinished action
- Urgent contact and record closure
- Use the clinic-approved training contact instructions; Records Role retains the practice summary using the normal manual process
The coordinator checks that the recipient understands how to find the plan and which questions still need a response. The reviewer confirms who owns the unscheduled follow-up. Staff then rehearse a handover to the cover reviewer.
Record any duplicated entry, unclear wording or unassigned action before approving the workflow. A completed handoff documents the process; it does not establish clinical recovery.
Check the handoff before sharing
- Confirm the clinician approved the patient-specific instructions and version.
- Check access instructions, the agreed update window and the owner questions that remain unresolved.
- Name the reviewer, cover and person arranging any unscheduled follow-up.
- Keep clinical decisions in the authoritative record and agree how the recovery summary will be preserved.
Connect the handoff to the recovery workflow
Vet Aftercare supports clinic-authored plans, owner check-ins and clinic replies in a shared recovery workspace. Review and adapt the plan for each patient before sharing access. Keep your existing records process: Vet Aftercare does not automatically synchronise with your PIMS or send this template as a message.
