When to use this tracker
Use it to organise who reviews an expected update, who covers an absence and what remains unfinished at shift handover. Start with one workflow your clinic already approves.
Choose a case reference meaningful to authorised staff. Put any patient-specific details only in your approved clinical record. The public page has no fields for entering or uploading clinic, owner or patient information.
Copy the blank case block
Repeat this block for each case in your approved local document. You can also use the field names as spreadsheet columns. Replace the placeholders locally; this website does not save or send a completed tracker. Use your browser’s Print command for a paper copy.
- Case reference
- [Internal reference in your clinic-approved record]
- Expected check-in window
- [Clinic-supplied day/date, start and end, and local time zone]
- Update or review status
- [Awaiting update / update received / needs review / handover open / complete]
- Reviewer and cover
- [Responsible staff role; cover role]
- Next administrative action
- [Describe the next action without copying clinical free text]
- Action owner and agreed due time
- [Role accepting the action; clinic-agreed date/time and local time zone]
- Record and handover route
- [Where authorised staff find the approved record and accept the handover]
A blank or incomplete field means the team still needs to agree it; it is not a software-generated decision. Choose terms that match your clinic's actual workflow. An update is not overdue merely because the calendar changed: use the clinic-approved window.
Use this template manually for as long as it helps. If a shared review workspace fits your team better, you can evaluate Vet Aftercare with a fictional case. Patient details and plans require manual setup; this template does not import automatically.

A complete fictional handover example
Training Clinic A and Practice Case A are fictional. The times below are administrative examples for a practice exercise, not a medical schedule or a recommended response target.
- Case reference
- Practice Case A, in the fictional training record
- Expected check-in window
- Example training day, 10:00-16:00, clinic local time
- Update or review status
- Practice update received at 14:00; needs review
- Reviewer and cover
- Reviewer A; Cover Reviewer B
- Next administrative action
- Acknowledge the practice note asking whether the update was received
- Action owner and agreed due time
- Reviewer A; the clinic-agreed staffed review period. If unfinished before handover, Cover Reviewer B accepts it and records the next agreed time
- Record and handover route
- Approved fictional training record; Cover Reviewer B confirms acceptance there
At 09:00, the example check-in window has not started, so the expected update is not overdue. At 14:00, the staff member acting as the owner submits the nonclinical note: Please confirm my update reached the team.
Reviewer A checks the practice record and posts an acknowledgement. If the task remains unfinished, the cover reviewer accepts responsibility and records the next action. If the exercise introduces a clinical question, the responsible clinician supplies the decision through the clinic-approved route. The tracker does not choose treatment or clinical urgency.
Finish a handover without losing the next action
- Check which expected updates are still within their agreed windows and which received updates need review.
- Give each unfinished action a responsible role and a locally agreed next review time. Confirm cover rather than assuming someone else will see it.
- Keep the response and any clinical decision in the clinic-approved record. Preserve or export the recovery summary using your normal records process.
- Review the template with the team after the fictional exercise. Remove duplicated entry and clarify any field that staff interpreted differently.
Where Vet Aftercare fits
The clinic keeps responsibility for care and review coverage. Vet Aftercare connects clinic-authored plans, structured owner updates and clinic replies in a shared recovery workspace. It can support the review process your team agrees, but it does not automatically import this tracker, synchronise with your PIMS or guarantee response times.
