Clinic workflow review resource

Find gaps in your clinic's recovery follow-up workflow

Use this free checklist to review how plans, owner updates and unfinished actions move through your clinic. Record what the approved sources show, choose a specific process change and check it again. This is an administrative workflow worksheet; the clinical team remains responsible for clinical audit design, care decisions and patient assessment.

Choose a clear review question

Start with one question your team can answer from its approved records, such as whether a cover role accepted an unfinished action. Agree the workflow and period being reviewed, who may inspect the source records and where the review will be retained.

Use internal references in the working document and keep patient details in the clinic's approved systems. This public page has no fields for entering records, no upload and no audit score.

Separate evidence from interpretation. A missing entry may leave a question unverified; it does not establish that an event did not happen or that a patient recovered. Decide what additional evidence is needed before treating the question as answered.

Download the follow-up review pack

The pack brings together the audit checklist and finding log, team rehearsal cards and an owner-understanding worksheet. It includes blank fields and fictional examples. Download it without an account or email address, then adapt the Word version or print the PDF.

Use the pack with your existing process. It does not inspect records, calculate a score, send messages or import a workflow into Vet Aftercare.

Check the handoffs against the record

For each item, record Observed, gap to address or not checked, and state the evidence behind that choice. These are administrative findings, not clinical ratings.

Approved plan
Can the reviewer identify the plan version and the source of the patient's approved instructions? Record where you checked, rather than assuming the latest reusable protocol applies.
Owner explanation
Is there a record of how access, the requested update and the clinic's contact route were explained? Keep unanswered questions visible.
Expected update window
Is the clinic-approved window and local time zone available to the reviewer? Distinguish an absent response from one whose agreed window has not ended.
Review responsibility
Can staff identify who currently owns the review action and whether proposed cover has accepted it? A role listed as cover is not evidence of acceptance.
Question and response
Can the team connect a reported question to the recorded response or next responsible role? Opening an update or acknowledging it does not establish that the question was answered.
Follow-up arrangement
Does the record distinguish follow-up needed, an arrangement being made and a confirmed appointment? Keep an unconfirmed booking explicitly not yet scheduled.
Summary and record destination
If a summary was prepared or exported, is its approved record destination known and checked? A file existing is not proof that the intended record received it.
Gaps and remaining work
Are missing information and unfinished actions stated with a responsible role? Avoid interpreting missing updates or completion counts as evidence of recovery.

Do not turn this checklist into a pass mark for clinical care. Keep the scope specific, explain incomplete evidence and route questions requiring a clinical decision to the treating team.

Use the same definition when you check the process again. If the review question or evidence source changes, record that change instead of treating the two reviews as directly comparable.

Copy a finding and improvement record

Complete one record for each finding in your clinic-approved document. Use the blank fields below or the editable review pack. Use your browser’s Print command for a paper copy.

Review reference and scope
[Internal review reference, workflow and period; keep patient identifiers in approved records]
Item being checked
[The agreed administrative question and evidence you need]
Evidence inspected
[Approved record reference, version and the specific entry checked]
Observation
[What the source actually shows, without inferring clinical meaning]
Finding
[Observed / gap to address / not checked; explain uncertainty]
Next change or action
[Specific action, current responsible role and accepted cover if needed]
Recheck method
[What you will inspect after the change and the team-agreed review point]
Result and remaining questions
[What the recheck actually showed; keep unverified items open]

Choose a change that follows from the observed gap. For an unclear handover, an explicit acceptance step may be more specific than asking everyone to communicate better.

Record the recheck result as it stands. A successful fictional exercise shows that the team followed that exercise; it does not establish ongoing use, time saved or a clinical outcome.

A fictional review with three different findings

Training Clinic J is reviewing administrative practice records. The cases, roles and exercise results are fictional.

Practice Case J: observed
The expected practice window is recorded and has not ended. Staff correctly retain awaiting update. No medical schedule or overdue threshold is supplied by this example.
Practice Case K: gap to address
A cover role is named, but the handover record does not show acceptance. The original reviewer remains responsible until the cover confirms.
Practice Case L: not checked
A practice summary file exists, but its intended record destination has not been inspected. The reviewer records the transfer question as unverified rather than claiming success or failure.

The three findings describe different evidence states. Keep not checked visible when the information has not been verified; do not convert it into a reassuring result.

A complete fictional improvement record

Review reference and scope
Training Review J; one fictional handover in the approved training record
Item being checked
Whether the cover role accepted an unfinished access-help action
Evidence inspected
Practice Case K handover entry lists Cover L but contains no acceptance
Observation
Reviewer K has named proposed cover. The record does not show that Cover L accepted the action
Finding
Gap to address in the handover record; no clinical outcome is assessed
Next change or action
Reviewer K retains responsibility and asks Cover L to confirm acceptance through the approved training process
Recheck method
During the next agreed practice handover, check for explicit acceptance and a clear next action
Result and remaining questions
In the rehearsal, acceptance is recorded and the next action remains visible. Check repeated use before drawing any wider conclusion

The team changes the training handover wording to require explicit acceptance, then repeats that part of the exercise. This small practice result is recorded separately from any later review of routine use.

Recheck the process and keep remaining work visible

  1. Agree the exact gap and the source evidence with the appropriate reviewer.
  2. Name one specific change and the role responsible for it.
  3. Rehearse the change with fictional information in an approved training environment.
  4. Recheck the same administrative question through the clinic's approved records process.
  5. Record what was observed, what remains unverified and the next responsible role.

Review the information in its workflow context

Vet Aftercare connects clinic-authored plans, owner check-ins, clinic replies and recorded follow-up activity. Your team decides what to review and how to interpret it. The software does not perform this audit, certify the process or establish recovery from a completion count.

Patient setup and protocol configuration remain manual, and recovery summaries do not automatically synchronise with your PIMS. Use your normal approved record process when reviewing or retaining information.
