Separate the reusable plan from the patient plan
Use the worksheet to prepare a consistent starting point for a workflow your clinic already approves. It is different from the discharge handoff, which records what was approved for one patient, and the follow-up tracker, which keeps the next administrative action visible.
A protocol template is not a prescription. The treating clinician reviews the instructions, questions, timing and follow-up arrangements for the patient. Do not insert generic treatment steps or recovery-day schedules from this worksheet.
Copy the protocol planning worksheet
Copy the fields into a clinic-approved document and complete them with your clinical reviewer. Keep patient records and private access links out of this public page. Use your browser’s Print command for a paper copy.
- Protocol identity and purpose
- [Internal name, intended clinic workflow, version and owner]
- Approved source and clinical reviewer
- [Authoritative instruction source, approving role and approval date]
- Use and adaptation boundaries
- [Clinic decides when this starting point is suitable and what must be personalised]
- Care-step structure
- [Clinician-authored step headings, instructions and patient-specific fields to review]
- Timing and follow-up fields
- [Clinician-defined timing, local time zone, appointment fields and not-yet-scheduled process]
- Owner check-in questions
- [Clinic-approved question wording and available answers; avoid duplicate free-text prompts]
- Requested media
- [What the clinic requests, if appropriate, and how staff review it]
- Review responsibility
- [Reviewing role, staffed coverage, cover role and clinical escalation route]
- Urgent contact wording
- [Clinic-approved contact and out-of-hours instructions]
- Version approval and later changes
- [Who approves a revision, its change note and where the previous approved version is retained]
Agree which wording is reusable and which fields staff must review for every case. Preserve the patient's approved plan and submitted history when a reusable protocol is revised; use your clinic's controlled update process.
An expected owner update needs a reasonable clinic-approved window. A date changing at midnight alone does not make the update overdue. The clinical team chooses the schedule; the worksheet does not calculate it.
Use the worksheet in your current process or evaluate the structure in Vet Aftercare. Staff configure the protocol and patient plan manually; there is no automatic worksheet import.

A fictional protocol planning example
Training Clinic A is rehearsing a protocol called Sample Follow-through A. It is a fictional administrative exercise, not a clinical protocol.
- Protocol identity and purpose
- Sample Follow-through A, version 1; used only to rehearse the training workflow
- Approved source and clinical reviewer
- Training Clinical Reviewer supplies and approves sample wording in the authorised training record
- Use and adaptation boundaries
- Training use only; the reviewer checks all patient-specific fields before the coordinator prepares a practice plan
- Care-step structure
- Sample care-step placeholders are completed by the clinical reviewer; no treatment text is supplied by this example
- Timing and follow-up fields
- Reviewer-supplied practice timing is copied into the sample plan. An unscheduled follow-up stays labelled not yet scheduled.
- Owner check-in questions
- The team reviews a nonclinical practice prompt asking whether the person acting as the owner can find the plan
- Requested media
- No media requested for this access-and-review exercise
- Review responsibility
- Reviewer A checks the practice update; Cover Reviewer B accepts unfinished work before handover
- Urgent contact wording
- Only the clinic-approved training contact instructions are used; no real contact or message is sent
- Version approval and later changes
- The reviewer approves version 1, records a proposed wording change as version 2 and retains the version 1 practice record
The coordinator prepares a fictional patient plan from the approved version. Another staff member follows the access instructions and submits a practice update. The reviewer responds, then the team checks whether the version and next responsible role remain clear.
Record what required manual re-entry and which instruction caused confusion. A successful exercise shows that the team can follow the process; it is not evidence of time saved or improved clinical outcomes.
Review a protocol revision with the team
- Identify the proposed wording or workflow change and its clinical approver.
- Confirm what applies to future plans and how any patient-specific change will be reviewed through the clinic's normal process.
- Check the owner questions, requested media, update window and urgent contact wording together.
- Rehearse the revised version using fictional information and preserve the approved history.
Use the structure in Vet Aftercare
Vet Aftercare lets clinic teams prepare reusable recovery protocols and personalise plans for individual patients. The clinic keeps responsibility for content and clinical decisions. Review the actual plan before sharing the owner's private access, and keep your normal PIMS record process.
